Validation of an Er,Cr:YSGG Laser-Assisted Endodontic Protocol using a Rapid, Chair-Side ATP Biomarker Test

Background and Limitations of Traditional Methods:

Bacterial testing of root canal systems in patients has traditionally presented numerous challenges. The widely used culture technique has not achieved widespread acceptance due to its arduous and time-consuming nature. DNA-based identification methods, such as polymerase chain reaction (PCR), are also problematic, as they suffer from high rates of false positive results because they detect DNA from dead bacterial cells. In addition, extrapolating findings from research conducted on extracted teeth with artificially created biofilm to clinical patient treatment can lead to erroneous inferences. While such research may provide valuable insights into the efficacy of various endodontic protocols, it does not address the limitations encountered during actual patient care. 

Introduction of the ATP Assay Method:

A newly introduced ATP assay method provides rapid identification of organic residues in root canal systems by detecting adenosine triphosphate (ATP) and its degradation products, adenosine diphosphate (ADP) and adenosine monophosphate (AMP), through a combination of luminescent reactions. This innovative method addresses a longstanding challenge in endodontics: the need for a swift, sensitive, and user-friendly tool to evaluate root canal cleanliness at the point of care. By allowing practitioners to quantitatively assess biological contamination in seconds, the test bridges the gap between conventional cleaning techniques and real-time clinical verification, potentially raising the standard for treatment quality and patient outcomes.

Clinical Application and Validation:

The ability to rapidly detect biological activity within canals enables dentists to assess and, if necessary, refine or repeat their endodontic protocol in real-time. This serves as an accurate indicator of the readiness of the root canal system for obturation. The ATP assay detection method will be presented alongside data validating an Er,Cr:YSGG laser-assisted endodontic protocol for cleaning and disinfecting root canal systems. Clinical cases will also be discussed, highlighting the role that the ATP assay test can play in managing a variety of clinical situations.

Upon completing this presentation, participants will have achieved the following objectives: 

  1. Participants will be able to describe an advanced, evidence-based protocol utilizing laser assistance for endodontic procedures. This protocol is specifically designed to enhance the predictability and reliability of endodontic outcomes, thereby providing practitioners with a greater degree of confidence in their clinical results.  
  2. Attendees will learn to implement a highly sensitive and rapid ATP biomarker test that can be conducted chair-side. This test enables clinicians to quickly and efficiently evaluate both the biological activity within the root canal system, and the overall cleanliness achieved following treatment.  
  3. Participants will be prepared to discuss and articulate how the ATP assay can serve as a valuable tool for validating endodontic protocols. Emphasis will be placed on integrating this test into clinical practice to ensure optimal treatment verification.
